Military Unit Organization Accounts

Required FRG Opening Account Documentation

FRG (Family Readiness Group. Checklist also used for “Cup and Flower” and similar accounts)

• DOD Memorandum from the Current Commander stating the following (must be specified)
  • Authority to open account at ACU
  • EIN#
  • FRG Name (much match IRS EIN documents)
  • Authorized signers names
  • Change of signers
  • Powers/duties to conduct banking services with ACU (i.e. types of accounts to open)
  • Debit Cards/Checks to be issued if applicable
  • Whether or not dividends will be forfeit
  • Current Commander’s signature
• DOD Assumption of Command Letter/Memorandum (references AR 600-20 authority & signed by CDR)
• TIN: IRS-issued EIN (Get an EIN here). Note: Name of Unit Organization must exactly match EIN registration.
• ID – State–issued DL/ID, passport of authorized signers

FRG (Family Readiness Group. Checklist also used for “Cup and Flower” and like accounts)

• DOD Memorandum from the Current Commander stating the following (must be specified)
  • Authority to update/change account at ACU
  • EIN# (must match current FRG account EIN)
  • FRG Name (must match current FRG account name)
  • Authorized signer(s) names to be removed
  • Authorized signer(s) names to be added
  • Final complete list of authorized signers remaining on the account
  • Debit Cards/Checks to be issued for new signers if applicable
  • Whether or not dividends will be forfeit
  • Current Commander’s signature
• ID – State–issued DL/ID, passport of authorized signers

If current Commander is different from last recorded Commander listed on account, also provide the following:

• DOD Assumption of Command Letter/Memorandum for current commander (references AR 600-20 authority & signed by Commander)
